• A notarized Power of Attorney from the Applicant. Legalization is not necessary (a fax copy is acceptable at filing provided that the original is submitted within 3 months from the filing date). One Power of Attorney can be used for plurality of trademarks if no particular mark are identified in the Power of Attorney;
• 20 trademark specimens with the size of each not larger than 80mm x 80 mm and not smaller than 15mm x 15mm (required at filling);
• A certified copy of the priority application and the English translation thereof (if the application is not in English). This document is applicable where the Paris Convention Priority is claimed and can be submitted later, after filing;
• A certified copy of the Regulations governing the use of the mark (where the mark is collective mark – required at filling).

For filing renewal

• A notarized Power of Attorney from the trademark owner. Legalization is not necessary (a fax copy is acceptable at filing provided that the original is submitted within 3 months from the filing date); 
• Original Certificate of Trademark Registration (required at filing); 
• Original endorsed Affidavit of Use/Non-use (required at filing);
• 20 specimens of the registered mark (required at filing).

For filing an affidavit of Use/Non-use

• A notarized Power of Attorney from the trademark owner. Legalization is not necessary (required at filing; a fax copy is acceptable provided that the original is submitted within 60 days from the filing date); 
• Original signed Affidavit of Use or Non-use. In case of Use Affidavit, evidences of use of the registered mark in Cambodia are required (such as bill of lading, packages, advertising and/or promoting materials, brochures, company profile, catalogs, photographs, etc.);
• Original Certificate of Trademark Registration (required at filing).

Note: One Affidavit can only be used for recordal of use or non-use of one registered mark in one class of goods or services.

For recordal of assignment

• A notarized Power of Attorney from Assignee. Legalization is not necessary (required at filing; a fax copy is acceptable provided that the original is submitted within 60 days from the filing date); 
• Original notarized/certified copy of the Assignment Agreement, including appendix or appendices (if any) plus a sworn English translation(s) if the documents are not in English (required at filing); 
• Original Certificate of Trademark Registration (required at filing).

For recordal of license

• A notarized Power of Attorney from Assignee. Legalization is not necessary (required at filing; a fax copy is acceptable provided that the original is submitted within 60 days from the filing date); 
• Original notarized/certified copy of the License Agreement, including appendix or appendices (if any) plus a sworn English translation(s) if the documents are not in English (required at filing); 
• Original Certificate of Trademark Registration (required at filing).

Note: The registration of a collective mark or an application therefore, may not be the subject of a license agreement.

For recordal of change of name/address

• A notarized Power of Attorney from the trademark owner. Legalization is not necessary (required at filing; a fax copy is acceptable provided that the original is submitted within 60 days from the filing date); 
• Original notarized Declaration of Change of Name/Address or legal documents showing the change of name/address;
• Original Certificate of Trademark Registration (required at filing).
